현미경분리냐? 육안분리냐?

모발이식수술 중 중요한 과정인 모낭분리는
모낭분리사의 숙련도뿐 아니라
현미경을 사용하는지 혹은 육안분리하는지에 따라
아래와 같이 2가지 관점에서 분명히 차이점이 있다는 것이 정설입니다.

첫째: 공여부의 같은 면적에서 얼마나 많은 모낭을 분리해서 이식부에 옮겨 심을 수 있느냐?
둘째: 머리카락을 얼마나 튼튼하게 분리하여 이식 후 생존율을 높일 수 있느냐?

(질문)

모발이식수술시 현미경을 사용하는 것이 좋은지 않은지 많은 논란이 있는 것 같은데요.
제가 목격한 바로는 현미경을 사용하는 병원이 그리 많지 않던데 현미경사용이 그렇게 중요한가요?

(답변)

제가 7년 전 발표했던 “현미경과 확대경을 사용한 모낭분리에 관하여..”
(Dissecting Microscope versus Magnifying Loupes)에서 언급했듯이

현미경을 사용하면 쓸데없이 버리는 모낭을 줄일 수 있는데
다시 말하면 공여부의 두피로부터 훨씬 생존력이 뛰어난 머리카락을 분리해 낼 수 있습니다.

쓸데없이 머리카락을 낭비하는 것이야말로 간과할 수 없는 문제입니다.

예를 들어 흰 머리카락의 경우 현미경을 이용해 분리하면 40%이상 많은 머리카락을 얻을 수 있습니다.

그리고 보통사람의 경우에도 모낭분리시 현미경만 사용할 경우 20~25%나 많은 양을 분리해 낼 수 있습니다.

탈모가 심한 사람의 경우 비효율적인 모낭분리로 인해 일단 머리카락이 없어지거나 손상된다면,
아까운 머리카락을 영영 돌이킬 수 없게 됩니다.

Q) There is a lot of talk about the need / not the need for microscopes in hair transplantation, but as I visited various groups performing transplants, only a few use the microscope. Is it really important?

A) We published an article about 7 years ago, (Dissecting Microscope versus Magnifying Loupes) which showed that using microscopes reduces waste, which means that more viable donor hair is available from every square inch of scalp that is harvested. The wastage is not a minor issue; those with white hair may find as much as 40% more hair can be obtained by using a microscope for dissection. The average person gets 20-25% more usable donor hair if a microscope is competently used in the dissection process. For those who are very bald, once hair is lost or damaged through inefficient dissection, that precious hair is never retrievable.
