There are a few problems with splitting up Proscar (5mg finasteride) tablets to get the correct dose for hair loss (Propecia; 1mg finasteride):
1) getting the wrong amount of medication:
Too much (potentially causing more side effects) or too little (not reaching the effective dose to protect your hair) is a problem. "More" is not necessarily better. Merck's rigorous dosing studies proved that 1mg is the most effective dose for hair loss, with the minimum amount of side effects.
The Proscar 5mg pill is pear-shaped, so there's really no way to accurately get 1/5 of the 5mg pill. If you split it into 1/4's you're still getting too much medication (20% more than you need each day). That adds up to be alot of extra medication over months and years.
Merck doesn't have to guarantee that there is the same amount of medication equally spread throughout the 5mg pill. Meaning, you may get a perfectly equal Proscar split, but each fragment has different amounts of medication.
2) it is not easy:
Splitting pills is a hassle. It is very clear from my patients... the more hassle, the less "compliance." That means if you are trying to 'save a buck' by splitting proscar, you are actually more likely to go off the medication. Simply put, it requires a great deal of discipline to split up the pills over the long term. It's kind of like joining a gym that's less expensive, but farther away--you are less likely to go workout as often as you should.
3) Is it the same?
No one really knows if a chopped up piece of Proscar (where the coating is broken, etc.) actually works the same way pharmacologically in the body. There's a ton of data on the effectiveness of Propecia 1mg (coated pills) but no data on the bits of Proscar. For example, what if changing the coating by splitting pills made the medication absorb faster? Would you have to change the timing or amount of the dose to compensate? As physicians (and scientists) we just don't know for sure.
The good news is that the data on Propecia's effectiveness is very clear. You can check the studies that were done on thousands of men:
90% of guys on the medication for five years looked the SAME OR BETTER than when they started.
75% of guys not on medication (taking a placebo) looked MUCH WORSE after five years.
The 'tried and true' treatments include Minoxidil (Rogaine), Propecia and Ultra-Refined Follicular Unit Hair Transplantation. Up and coming therapies potentially include low level laser therapy, nutritional modification and, maybe someday, hair multiplication (hair 'cloning').
