I would like to know if Nizoral could possibly have any bad interactions with either monoxidil or finestaride. I don't understand how can Nizoral help against MPB, what does it do, lower DHT, promote growth? I live in Canada, and I was able to get Nizoral 2% over the counter. Dr Richard Lee, MD answered: "There are no contraindications to using Nizoral 2% shampoo or cream in conjunction with Rogaine 5% and Propecia. These are all complementary medications with combined benefits. Ketoconazole, the active ingredient in Nizoral, helps prevent the synthesis of DHT in the scalp. Since the finasteride only prevents the production of DHT by the Type 2 5-alpha reductase primarily in the prostate, it makes good sense to prevent the continual synthesis of DHT in the scalp."

Dr Richard Lee, MD answered: "Since (almost) everyone shampoos routinely, you can simply use a ketoconazole shampoo, e.g. Nizoral, to keep your full head of hair. That would be the simplest treatment plan. And, hopefully, that may be all you ever need to do. Ketoconazole shampoo has been proven to be beneficial in treating MPB by inhibiting the synthesis of DHT in the scalp. If MPB progresses despite the use of ketoconazole shampoo, then consider adding topical minoxidil to your regimen."

Firstly I would like to personally thank-you for your contribution to this forum and I am sure that most other hairloss sufferers appreciate the thoughtfulness and effort you put into your answers. If I was living in the States there would be no question that I would be one of your patients. I have tried to order your minox formulae but it was held up by customs in this country – god knows why. My question is with regard 2% ketoconazole shampoo which you recommended should be used by a young man in an earlier post. How many times do you need to use it a week and can you use a conditioner afterwards? Ta once again."

Dr Richard Lee, MD answered: "Ketoconazole shampoo was patented in the U.S. in June 1982 as an anti-dandruff shampoo. As early as 1988, researchers in Belgium (Dermatology. 196(4):474-7) postulated that ketoconazole might be beneficial in the treatment of MPB. Ketoconazole is a broad-spectrum antifungal agent, which impairs the synthesis of ergosterol, a vital component of fungal cell membranes. This same pharmacological action inhibits the synthesis of DHT in the scalp, which is why ketoconazole shampoo is beneficial in the treatment of MPB.
Apply a small amount of the ketoconazole shampoo onto the wet hair and work it into a lather, which you can gently massage onto the scalp. Allow the lather to remain on the scalp for at least five minutes before rinsing off. Ketoconazole shampoos should be used two - three times a week. If you are accustomed to shampooing on a daily basis, use some other shampoo on the days you do not use a ketoconazole shampoo.
Since ketoconazole shampoos are fairly harsh on the hair shafts, I’d recommend the routine use of a conditioner after cleansing the hair with ketoconazole shampoos. By trial and error, you should be able to find the proper shampoos and conditioners to give you the optimal cosmetic results while you simultaneously treat your MPB."
