또 hairlosshelp에서 놀다가 퍼 왔습니다.
얼마전에 이야기를 보고서 여기에 올릴까 하다가 처음 프로스카를 시작하시는 저같은 초짜
분들이 약효에 대한 걱정을 하실까봐

그냥 두려고 하다가, 그래도 좀더 많은 프로스카에 대한 정보를 알아두시는 것이 도움이
될 것 같아서 올립니다.

그리고, 제가 처음으로 프로스카를 시작할 때는 정말 아무것도 몰라서 너무 답답했었는데,
혹시나 지금 시작하시는 분들을 위해서 제가 모아둔 자료를 올리겠습니다.

(그리고, 지금은 보이지 않는 카린님이 궁금하네요.
이글을 보고 어떻게 생각하실지....

ive been through trama and alot od stress. i take hair vits and viviscal just for healthy
hair. i need to know if propecia is having a badf effect on me. im thinking if i didnt
start, i wouldnt have this problem and my hairline will still be straight on one side. if
spiro is right for me, how can i get it?????"

일단 burning을 어떻게 옮겨야 할지 모르겠습니다.
그냥 편의상 (두피의) 화끈거림으로 옮기겠습니다.

일단 질문의 요지는
머리카락의 탈색(머리카락의 색깔이 옅어지고 있다가 정확한 표현인 것 같습니다.)과 더불어
두피의 화끈거림에 관해서 물어보고 있습니다.

프로페시아를 지난 9개월간 복용을 했었는데,
복용초에는 굵은 머리카락들이
어느 날인가 갑자기 두피의 화끈거림과 함께 이전까지는 겪어보지 못한 심각한 shedding을
겪으면서 가늘어 지고 헤어라인 후퇴하고 있다고 적고 있습니다.

나머지는 지금 사용하고 있는 약들에 관한 이야기, 그리고 spiro가 지금의 상황에 도움이 될
것인지에 관해서 묻고 있습니다.

그리고 지금의 악화된 상황이 프로페시아에서 원인이 있지않나 의심하고 있다고 쓰
고 있습니다.

Dr Richard Lee, MD answered: "There is an article by Hoss in the March 1998 issue of
Archives of Dermatology , which describes "Scalp Dysesthesia". This may be your
problem. Some individuals who develop a telogen effluvium report painful burning
sensations in association with excessive shedding of the hair. Until recently this problem
was not officially identified by dermatologists. The syndrome has been called “scalp
dysesthesia” or “burning scalp syndrome”. The cause of burning scalp syndrome is
unknown. It has been successfully treated by some dermatologists with antidepressants
such as doxepin or amytryptaline. Topical spironolactone will not be of any benefit to
you. Please consult a local dermatologist. You may need to refer the dermatologist to
the article in the Archives of Dermatology, because very few doctors would be
knowledgeable about "burning scalp syndrome"."

Dr. Lee에 따르면, 이런 증상을 "scalp dysesthesia" (or "burning sydrome")고 부르며,
휴지기 탈모 (telogen effluvium)가 발전된 것으로 심각한(과도한) shedding과 함께 고통스
러운 화끈거림을 느낀다고 보고되었다고 합니다.

아직 피부과에서는 공식적으로 확인된 것은 아니며, 그 원인도 아직 밝혀지지 않았다고 합
니다.
하지만 몇몇 피부과 의사들에 의해서 항우울증 약품(antidepressants) - doxepin,
amytryptaline -으로 성공적으로(successfully) 치료되었다고 적고 있습니다.
(물론 이런 상황에서 spino는 별로 도움이 안 될 것은 당연한 이야기일 것입니다.)

그리고 당연한 이야기이지만, 극소수의 의사들만이 여기에 대한 (치료) 지식을 가지고 있으
므로

고 있습니다.

그리고 Finasteride의 shedding에 대한 좀더 자세한 자료도 www.minoxidil.com에서 퍼왔

There have been multiple reports of excessive shedding several months after finasteride
therapy. Typically, there is a good response to finasteride to prevent or reverse MPB.
Then, around the 11th to 16th week, there can be sudden shedding, sometimes on a
massive scale. The entire phenomenon fits the description of a telogen effluvium. It is a
common observation that post-partum women often suffer the same temporary hair loss.
In the case of finasteride use, the telogen effluvium appears to be a reaction to the
sudden change in the systemic levels of the sex hormone, DHT. Often the cause of a
telogen effluvium are obscure, but has been related to high fevers, stress, trauma,
medications, etc.

여기서 중요한 이야기는

갑작스러운 성 호르몬의 변화(dht같은..)로 휴지기 탈모가 일어날 수 있으며,
발생시기는 11주에서 16주 정도쯤에 일어나는 것으로..
그리고 여기서는 good response라고 소개하고 있습니다..(저는 finaseride의 shedding은
minoxidil의 shedding과는 발생기전이 다르므로 오히려 좋지 않은 징후라는 것으로 알고 있
습니다.)

The shedding is generally diffuse (global) and can affect areas of the scalp not usually
affected by MPB. So, it would be common to note shedding from the sides and back of
the head in addition to the crown, vertex and frontal areas. The shedding tends to be
fairly symmetrical, but will be more noticeable in the areas affected by MPB, because
there is a higher ratio of hairs in the telogen phase than in the other areas of the scalp.

그리고 남성형 탈모의 영향을 받는 곳(두정부와 전두부) 뿐만 아니라 좌우측두, 그리고 후두부
에서도
shedding의 영향을 받으며,
물론 남성형 탈모의 영향을 받는 곳에서 더욱 뚜렷이 관찰 되는 것으로,
그리고 좌우대칭적으로 shedding이 발생한다고 소개하고 있습니다.

The duration of a telogen effluvium is variable, but rarely lasts more than a few
months and there is invariably complete restitution unless another pathologic process
also occurs.

그 기간은 몇 달을 지속하지 않는 것으로(a few month라면 한 두달 정도를 가르키는 것 같
습니다.)
그리고 다른 병리(이상)이 없으면 틀림없이 (머리카락을)되 찾을 것이라고 적고 있습니다.
(하지만 예전의 머리카락이랑 똑같은 머리카락을 기대한다는 것은.......)

As a rule, treatment is not necessary because the hair will grow back. For most
patients, there is no evidence of residual loss of hair within a year. However, there have
been cases of patients taking finasteride and reporting repeated bouts of excessive
shedding. In this situation, it would be advisable to discontinue use of finasteride in
favor of alternative anti-androgens.

일반적인 경우에는 머리카락이 다시 자라지만, finasteride에 의해 반복적으로 과도한 shedding이 일어나는 경우에는 복용을 중지하고 다른 anti-androgen을 사용하라고 충고하고 있습니다.
