私も別に勉強も兼ねて、そして植毛に対する海外情報を共有して意見も交わすために連載文を載せたいと思います。足りない部分が多いと思いますので、参考にしていただければ幸いです。

本稿は、世界的に有名な植毛、権威者と患者が活動するhairsiteというコミュニティから抜粋して意訳した文です。

The shock loss phenomenon

When implantation takes place among existing hairs, the number of placed grafts per cm2 should be the one that the existing density permits, in order not to have the 멻air loss phenomenon�.

The hair loss phenomenon is about losing existing hairs of the recipient area and it may be the result of two problematic conditions:

1) Antagonism between the newly implanted follicles and the existing ones in the name of survival.

This condition may occur in case of excess density after implantation. Τhe newly implanted follicles are the ones that will survive, leading existing follicles to hair shedding.

Practically the existing hairs experience a metabolic shock that leads them to telogen phase. Τhis is caused by the demanding survival process of the newly implanted hair follicles.

2) Crossing thus traumatizing the existing follicles, by implanting new follicles.

This condition may also occur in case of excess density, after the new hairs implantation, or in case of false angle of placement.
The traumatized follicles shed hairs that may not re-grow.

In the hair shock loss phenomenon, there may be no re-growth of the shed hairs, since the follicles may endure un-restorable tissue damage.

For these reasons, during implantations for increased hair density, we are obliged to follow the existing angle and direction of hairs and we have to be careful with the implantation density.

This is also the reason why it is crucial for the patient to use minoxidil post procedurally, for a period of at least 6 months, in order to strengthen the existing follicles and help them survive through the new condition.

It has been observed that the shock loss phenomenon is more prone to occur in original hairs, than in previously implanted hairs.

If the follicle is not seriously injured, it can regrow from the tissue that has remained in place, under the condition that the rising of the hair is not inhibited by the fibrosis, caused through normal healing process.
That is why the regrowth percentage is not impressive.

Essentially this is where regrowth factors such as PRP, come in.
Their role being, a)to encourage good quality healing in the dermis, thus minimizing all relative factors, that may prevent the new hair from rising and b) to assist (this is still under research)- where possible- the regeneration of hair, from the remaining hair follicle stem cells. The shock loss phenomenon 既存毛の間に移植をする場合、脱毛現象を避けるために2cm当たりの移植毛は既存毛が許容する密度範囲でなければならない。
脱毛現象は移植部位が既存の毛脱落を意味し、以下のような2つの問題から寄与される。

1. 新たに移植した株と既存の株の対立この現象は、移植後の過度な密度によって発生する。生き延びようとする新しい株は生き残るようになり、既存の株を脱落させる。
事実上、既存の毛包は代謝性ショックによって休止期に入る。
このような現象は、新たに移植された株が生存しようとする過程で発生する。

2. 移植毛包が移植され、既存の毛包に外傷(衝撃)を週する場合、この現象もまた過度な密度または誤った移植角度によって発生する。衝撃を受けた株は、二度と育たない毛髪を脱落させる。 Hair shock loss phenomenonで毛包が回復が不可能な組織損傷に耐えるため、脱落した毛髪は再び育たないことがある。

このような理由から、高密度のための移植の際、既存の毛の角度と方向に従わなければならず、移植密度について注意しなければならない。

これが患者が手術後少なくとも6ヶ月間、ミノキシジルを使用して既存の毛包を強化させ、新しい環境で生き残るようにすることが重大な理由である。

衝撃による脱落現象は、以前に移植された株より既存の毛包で発生する傾向が観察された。もし既存の毛包が深刻に損傷したのでなければ、毛髪が育つのが正常な治癒過程で繊維症によって抑制されたのではないという仮定の下、毛髪は生き残った組織で新たに育つことができる。

これが、株の再生率が印象的でない理由である。根本的にこの時がPRPのような再生要素が役割を果たす時だ。その役割は、皮膚の再生を促進させることで、新しい毛髪が育つことを防ぐ本であれ、関連した要素を最小化させることだ。

結論: 簡単に見ると、地面があるのにあまりにも細かく種をまくと、お互いの栄養分を奪っていきながら結局片方が死んだり、両方とも健康でない状態で育つことを意味するようです。

Traumatizing部分は移植時(採取中も同様のようです)、既存の株の角度や方向をきちんと見ずに損傷させる場合を意味します。

結局、あまりにも高密度に植えると、新しい株が生き残ろうとする過程で既存の苗を押し出すというようですね。
そして、既存の毛包に近く植えられ、既存の毛包の損傷を意味することもあります。

ですから、毛髪移植時の密度が正常密度に及ばないと寂しがらないでください。植毛の限界です。
